The Change Foundation is investing in area family caregivers.
It’s launching a program today at Stratford General Hospital called Connecting the Dots…Smoothing the Transitions for Family Caregivers. $2.3 million is going to the project which is for people in both Perth and Huron counties.
The project is focused on addressing the needs of family caregivers by defining and recognizing their role and improving quality of care. You can find more information about Connecting the Dots at this website.
